Your first day

  • Confirm badge access to the CVICU, the fellows office and the 6N call room.
  • Confirm logins for Cerner, UConcall and the monitoring and reporting systems you will be asked for.
  • Find the ultrasound, the arterial-line supplies, the central-line and PAC kits, the temporary pacing equipment and the emergency equipment.
  • Review the current census, today’s attending, overnight events and pending procedures before rounds.
Need details

Where the CVICU work area and the fellow workstation are, how to get to them, and where each piece of equipment above is actually kept.

Answered by: Program

Roles & responsibilities

  • Provide cardiology consultation on ICU patients as requested.
  • Evaluate and manage unstable cardiac patients using clinical, ECG, telemetry, imaging and hemodynamic data.
  • Assist with scheduling CVICU cath and TEE procedures.
  • Assist with PA catheter, arterial line, IABP, TEE, ECMO and other supervised procedures.
  • Teach residents and ICU staff, and take part in end-of-life and family discussions.

Coverage & handoff

  • CVICU and Consults alternate ownership of the 4–6 PM block.
  • The late fellow owns new consults and CVICU admissions for those two hours.
  • Friday’s 24-hour CVICU call covers both services overnight.
  • No routine new consults after 6 PM unless urgent or emergent.
  • Post-call: finish essential notes and the handoff, then leave. 10 AM is the maximum, not the expectation.

The team around you

  • Cardiology attendings, fellows, residents and APPs; ICU nursing, pharmacy and respiratory therapy.
  • Interventional cardiology, EP, heart failure and cardiac surgery.
  • Nephrology, neurology, pulmonary and critical care.
  • Palliative care, social work and ethics.
  • Transitions out of the ICU are made as a team, not handed off by one person.

Escalate immediately

  • Cardiogenic or RV shock
  • STEMI or mechanical MI complication
  • Tamponade
  • Massive PE
  • Aortic dissection
  • Acute severe valvular lesion

Still needed from the program

Listed rather than guessed. These are the facts this page cannot answer.

  • LocationThe exact CVICU work area and fellow workstation, and how to get to them.
  • ConferenceWhether the CVICU fellow attends noon conference on the Friday of their 24-hour call. It is drawn on all five weekdays because it is the fellowship’s standing hour, not this service’s.
  • Late blockWhether Monday and Wednesday are fixed for the whole block, or move with the schedule. The matrix reads the real week either way.
  • CT surgeryThe current emergency cardiac surgery contact, and where its coverage is published.
